Well your form looks nice and is pretty comprehensive.

But a 15 page assessment form a coach has to fill out for each player?

Just how did you visualize this being used during a typical coaching session?

How much time with each player will the coach have?

That's a long check list to fill out during a coaching session if your coaching several players at once and have to fill it out/update one for each player every session.

or is it just to be filled out/updated after a certain number coaching sessions?

Some of that stuff IMO looks more like it should just be a separate form for the bowler or coach to fill out at the start of coaching, it's just back ground info on the player.

With the actual assessment form pared down to a shorter form a coach could easily check off and amend notes to when working with the individual players during a coaching session.

Might not have said that right, something about it just seems overly long.

It's just I imaging I see a coach with 8 players on his pair of lanes, with a stack binders piled up on the table getting ready to fall over. (Yes all the players sheets could be in one big binder, it's just the visual that popped up.)

As opposed to the coach just carrying a clip board with maybe a 2 page sheet for each player. That he can check and make notes on at the lanes, then maybe go back afterwards and use to update a master file.

Yes I agree, probably too long. My original idea was that it could be used as an initial evaluation at the beginning of the season and it would then be put into an individual binder (each player would have one) so that additional notes could be taken every subsequent practice. The initial evaluation would have to be a video analysis done by the coach in his or her own time after practice, probably at home, analyzing the athlete's video. Not all questions would have to be answered, this would all depend on the bowler's level and needs. I also was given the suggestion of dividing the document into different parts.
