When Mo says this does he mean 1st joint nearest tip of index finger, or 1st joint where the index finger connects to the hand?

The latter. You should feel most of the weight of the ball on the pad at the base of the index finger. It helps set your fingers left from the get go and makes it a little easier to get around the ball at the bottom.
